Entrant: MJHS Institute for Innovation in Palliative Care and Harbinger Group
Entry Title: Empowering Social Workers in Palliative Care

A national, evidence-based training program that combines self-paced learning, live instruction, and mentorship to strengthen social workers’ competence in palliative and end-of-life care. The program helped social workers apply their learning in practice, strengthening their ability to provide more coordinated, patient- and family-centered support.
